Low Testosterone Levels and Treatment

March 19, 2020

Blood sample tube with normal testosterone

Testosterone is the main male hormone, just as estrogen is the main female hormone. That said, males have a small amount of estrogen in their bodies, and females likewise have some testosterone. The high level of testosterone makes males look and sound the way they do: strong muscles, deep voice, facial hair, and so on are all due to this hormone.

In men and boys, testosterone is continually produced by the testicles and fosters healthy red blood cell production, mood stability, cognitive ability, and strong bones. If you are a man and your testosterone levels get too low, you will experience symptoms of low-T: low sexual desire, reduced muscle mass, weight gain, decreased energy levels, and impaired cognitive ability.

Testosterone levels begin to drop in men as a normal part of the aging process starting in their 20s. However, there are certain health conditions that can increase the decline in testosterone levels – and these underlying health conditions should be addressed first before beginning testosterone replacement therapy.

Treating Low-T with Testosterone Replacement Therapy

Testosterone replacement therapy is the treatment used to return a man’s testosterone levels back to normal. If your doctor recommends testosterone replacement therapy, it can be done via a variety of different methods:

Topically

Testosterone therapy treatment is available in creams, gels, liquids, and patches. Liquid, cream, and gel preparations are applied directly to the skin of the shoulders, upper arms, and/or thighs. Wash your hands thoroughly with soap after applying, and do not wash the application area until it’s time for the next dose.

For maximum safety of others, skin patches are available for males who do not want to accidentally transfer the hormone to those they touch. Skin patches are applied every 24 hours, and this treatment releases controlled amounts of the hormone into the skin for absorption into the man’s system.

Orally

Testosterone capsules are not swallowed like a pill but rather disintegrate in your mouth. The capsule should be placed in between your inner cheek and gums two times a day to facilitate the absorption of testosterone into the bloodstream.

Pellet Implantation

Testosterone pellets are surgically placed by a doctor under your skin. The location is usually in the hips or the buttocks. The pellets will release testosterone slowly, and they should be replaced every three to six months.

Intramuscular Injection

For long-acting testosterone therapy, your doctor will inject it directly into the thigh muscle every one, two, or four weeks. Your testosterone levels will rise considerably in the days following the injection and will then taper off. Discuss any unusual side effects with your doctor.

Men who receive testosterone replacement therapy usually experience an improvement in symptoms within a month after starting therapy. Your doctor will advise you regarding how long to continue the therapy.
